Making a referral
If you wish to discuss a referral, please contact us directly on 9329 4330.
We will need a completed referral form so that we have enough information to determine whether the case/request is appropriate, to discuss it at our Intake meeting, and to determine the priority of the referral. Incomplete or absent referral forms will slow down the process.
What sort of service do you need?
Sometimes people are unsure precisely what they need. Diverge offers neuropsychology assessments, behaviour assessments, behaviour support interventions, secondary and tertiary consultation, counselling and more. Please read our SERVICES section to help you decide what you need and the specific issues you want addressed.
