Making a referral

We need a completed referral form so that we have enough information to determine whether the case/request is appropriate, to discuss it at our Intake meeting, and to determine the priority of the case/request. Incomplete or absent referral forms will slow down the referral/request.

Who do we see?
We specialise in working with children and adults with Acquired Brain Injury (e.g due to head injury, stroke, hypoxia, infection, tumour, substance related brain injury, degenerative neurological conditions), their families and support teams. If you want to refer someone with intellectual disability, autism or other longstanding conditions since birth, please call or email us to discuss whether we are the most appropriate service for your needs before sending in a referral form.

What sort of service do you need?

Sometimes people are unsure precisely what they need. Diverge offers neuropsychology assessments, behaviour assessments, behaviour support interventions, secondary and tertiary consultation, counselling and more. Please read our SERVICES section to help you decide what you need and the specific issues you want addressed.

We encourage you consider this information, complete as much of the form as you can, submit it to us, and then we can discuss the referral with you by phone.

For NDIS participants, please ensure you have read the Diverge NDIS Policy 2019 relating to the services we can and cannot provide, and complete the NDIS Additional Required Information form in addition to the referral form.
