Making a referral

Due to an influx in new referrals, we have reluctantly decided to close our waitlist to new referrals. We will review this decision at the end of May 2021.

If you wish to discuss a referral, please contact us directly on 9329 4330.

When the waitlist re-opens, we will need a completed referral form so that we have enough information to determine whether the case/request is appropriate, to discuss it at our Intake meeting, and to determine the priority of the referral. Incomplete or absent referral forms will slow down the process.

Who do we see?

We specialise in working with children and adults with Acquired Brain Injury (ABI) their families and support teams. For example ABI due to:

  • head injury
  • stroke
  • hypoxia, lack of oxygen to the brain
  • infection
  • tumour
  • substance related brain injury
  • degenerative neurological conditions

If you want to refer someone with intellectual disability, autism or other longstanding conditions since birth, please call or email us to discuss whether we are the most appropriate service for your needs before sending in a referral form.

 

What sort of service do you need?

Sometimes people are unsure precisely what they need. Diverge offers neuropsychology assessments, behaviour assessments, behaviour support interventions, secondary and tertiary consultation, counselling and more. Please read our SERVICES section to help you decide what you need and the specific issues you want addressed.

 

Referral Form

When the waitlist re-opens, you will be able to complete the referral form online or you can download the referral form and send back to us via email or post.
