Making a referral
We need a completed referral form so that we have enough information to determine whether the case/request is appropriate, to discuss it at our Intake meeting, and to determine the priority of the referral. Incomplete or absent referral forms will slow down the process.
Who do we see?
We specialise in working with children and adults with Acquired Brain Injury (e.g due to head injury, stroke, hypoxia, infection, tumour, substance related brain injury, degenerative neurological conditions), their families and support teams. If you want to refer someone with intellectual disability, autism or other longstanding conditions since birth, please call or email us to discuss whether we are the most appropriate service for your needs before sending in a referral form.
What sort of service do you need?
Sometimes people are unsure precisely what they need. Diverge offers neuropsychology assessments, behaviour assessments, behaviour support interventions, secondary and tertiary consultation, counselling and more.
